japan visa requirements
ORIGINAL and in A4 SIZE PAPER unless otherwise stated
1. PHILIPPINE PASSPORT
- More than 6 months valid after the travel date
- Must have signature
- Lamination of picture portion should not be broken
- At least 2 blank pages available
2. VISA APPLICATION FORM
- completely filled and signed
- application date indicated
- put N/A in all spaces that are not applicable, especially page 2
- with complete hotel name/address/contacts and name of airline
Japan Embassy will not accept forms with blank, no signature, no date, with erasures, and may deny applications not filled out correctly or with wrong information
3. PHOTO
- 1 copy, 4.5cm x 4.5 cm, with white background
- must be taken within 6 months from date of application
- name and birthdate of application should be written at the back
- must be attached in the application form
4. BIRTH CERTIFICATE
- NSO/PSA issued, original with receipt, valid 1 year
- If Birth certificate is late registered (c) or no record (b), or unreadable (a)
(a) submit photocopy from civil registrar's office
(b) submit original copy of no record + submit photocopy from civil registrar's office
(c) photocopy of baptismal certificate (with complete address and landline number of the church in the Baptismal Certificate) + photocopy of school record (Form 137, with complete address and landline number of the school)
- with used Japan visa before and is not sponsored/guaranteed, exempted from submitting birth certificate
5. MARRIAGE CONTRACT if married
- NSO/PSA issued, original with receipt, valid 1 year
- with used Japan visa before and is not sponsored/guaranteed, exempted from submitting marriage contract
For 4 and 5, in case of non-record of marriage, submit certificate of non-record together with the one from local civil registrar
6. TOUR ITINERARY
- Prepared by applicant or travel agency
7. BANK BALANCE CERTIFICATE
- or copy of bank book for 2 years,
- or Original bank certificate, valid 3 months from date of issue
8. Photocopy of Certificate of Employment and photocopy of ITR from your current company (2316/1700/1701/1702)
- if submitting company ITR, include photocopy of DTI/SEC
9. INCOME TAX RETURN
- Form 2316 or 1707, original and photo copy
- if no ITR, write an Explanation letter to the Embassy
- for senior citizens, submit a photocopy of Senior Citizen ID
10. For sponsored/guaranteed applicants
- submit an accomplished Guarartee Letter form. Provide a proof of relationship through Original Birth and/or Marriage Certificate
11. Affidavit of Discrepancy - if the names or birth dates are misspelled across all your documents
12. Authorization letter - authorizing The Agency to process your visa application
13. Request for Multiple-entry visa form
14. Copy of I-CARD or AEP for foreign Nationals
